It's important to note that you are not obligated to take these substances, but if you choose to do so, you should follow the instructions and recommended doses. In my participation in Rooted Path's Integrative Health Counseling Services, I hereby accept all risks to my health and hereby release the Specialist and Rooted Path, on my behalf and on behalf of my personal representatives, estate, heirs, next of kin, and assigns, from all costs, claims, causes of action, and damages arising from any illness or injury to my person, including my death, that may result or occur as a result of my participation in Rooted Path's Integrative Health Services, whether caused by negligence or otherwise.
